Founding Member Community Support Program



Opening a nurse-led primary care clinic takes vision, preparation, and community support. The Founding Member Community Support Program allows individuals and families—at every income level—to participate in building accessible, patient-centered healthcare in our community.

Support is completely voluntary, and every dollar donated is appreciated. Whether someone contributes $5 or $5,000, each supporter is recognized and valued.

Participation in this program is never required to receive medical care.



🤍 Community Supporter — $25


A meaningful way to say “I believe in this.”


Your support helps cover the many small but essential costs that go into opening a clinic—supplies, preparation, and behind-the-scenes work that makes care possible.

Includes:

  • Recognition as a Founding Member (private unless public recognition is requested)
  • Inclusion in Founding Member updates and clinic communications
  • Our sincere appreciation for being part of this beginning

🌱 Community Ally — $50


Helping move the mission forward..


At this level, you are helping lay the groundwork for accessible, compassionate primary care in our community.

Includes everything above, PLUS:

  • Early updates on clinic progress and opening milestones
  • Recognition as a Community Ally (private unless public recognition is requested)

🌿 Founding Ally — $100


Standing with us as we build.


Your support helps strengthen the foundation of the clinic and ensures we open in a way that prioritizes quality, not quantity.

Includes everything above, PLUS:

  • Priority notification when patient onboarding begins
  • Recognition as a Founding Ally (private unless public recognition is requested)

🔵 Founding Supporter — $250


Helping turn vision into action.


This level of support directly assists with startup needs that allow the clinic to serve patients from day one.

Includes everything above, PLUS:

  • Early access to appointment scheduling once the clinic opens
  • Recognition as a Founding Supporter (private unless public recognition is requested)

🟣 Community Advocate — $500


Investing in access and dignity.


Your contribution helps support the administrative and operational needs that make it easier for patients—especially those navigating barriers—to access care.

Includes everything above, PLUS:

  • Priority invitations to community education or wellness events
  • Annual clinic impact update
  • Recognition as a Community Advocate (private unless public recognition is requested)

🔴 Legacy Partner — $1,000+


Leaving a lasting mark on community health.


This level of support helps shape the future of care in our community and strengthens long-term sustainability.

Includes everything above, PLUS:

  • Opportunity to support a clinic initiative or community health effort
  • Annual written impact summary
  • Recognition as a Legacy Partner (private unless public recognition is requested)

⚖️ Transparency & Compliance

  • Founding Member support is voluntary and one-time
  • Participation is not required to receive medical care
  • Contributions do not influence medical decision-making
  • All medical services are billed according to insurance or standard payment policies
  • Founding Member benefits are non-clinical and administrative
  • Contributions support clinic startup and community health access
