We provide comprehensive primary care services including preventive care, acute visits, chronic disease management, medication management, and routine health maintenance—both in our clinic and in the comfort of your home.
We offer in-home Medicare Annual Wellness Visits, regardless of who your primary care provider is. These visits focus on prevention and safety, including:
Health risk assessments
Medication review
Fall risk and safety screening
Cognitive and depression screening
Personalized care planning
Patients do not need to change their doctor to receive an in-home AWV from Next Level Healthcare PLLC.
After hospitalization or skilled nursing stays, we provide Transitions of Care services to support safe recovery and reduce readmissions. Services include:
Post-discharge follow-up visits (home or clinic)
Medication reconciliation
Patient and caregiver education
Coordination with hospitals, specialists, and primary care providers
These services can be provided even if we are not your PCP.
For patients with two or more chronic conditions, we provide Chronic Care Management with monthly, or biweekly, check-ins from our care team. CCM helps:
Improve symptom control
Prevent complications
Reduce ER visits
Support caregivers and families
This service supports patients between primary care visits and works alongside your existing provider.
We integrate mental and behavioral health support into primary care, including:
Depression and anxiety screening
Care coordination
Monthly Check-ins
Same-day connections to mental health professionals when available
